- As an adjunct to prescribed diet and exercise for the management of blood glucose concentrations in non-insulin dependent diabetic patients who are inadequately controlled by diet alone or by diet and oral hypoglycemic agents.
- The optimal dosage must be individualized.
- ARBO tablets should be swallowed whole with a little liquid directly before a meal or chewed with the first few mouthfuls of the meal.
- In adults, ARBO should be started at a low initial dose and be increased slowly to minimize the gastrointestinal side effects. Treatment is usually commenced at 25 mg once daily for the first week, 25 mg twice daily for the second week and 25 mg three times a day for the third week. A further increase of the dose may be necessary after 4 – 8 weeks on the basis of the blood glucose level.
- The average adult dose is 50 mg ARBO three times daily. A further increase to 100 mg ARBO three times daily may occasionally be necessary. The doses are adjusted weekly if needed and tolerated.
- The best candidates for the use of ARBO is the patients who have higher postprandial glucose levels but it is not recommended for patients with intestinal and liver disease.
- ARBO is contraindicated in patients with severe renal impairment (creatinine clearance < 25 mL/min).
- ARBO should not be used in patients with gastrointestinal disorders associated with malabsorption. It should not be used in patients with inflammatory bowel disease, such as ulcerative colitis and Crohn’s disease.
- ARBO should not be used in patients with partial intestinal obstruction, or in patients predisposed to intestinal obstruction or ileus. Similarly, ARBO should not be used in conditions that could be aggravated by an increased formation of intestinal gas (e.g, Roemhelds syndrome, major hernias, intestinal obstruction and intestinal ulcers).

- Gastrointestinal symptoms: In placebo-controlled trials, the incidences of abdominal pain, diarrhea, and flatulence were 19%, 31%, and 74% respectively in 1255 patients treated with Acarbose 50–300 mg tid.
